Sanjivani Individual- Schemes

  • INTRODUCTION:
    Individuals can take membership. The rules & Privileges in case of Individual membership shall be as under.
  • CATEGORY OF MEMBERSHIP:
    Any Individual can take membership in any one of the following categories: -
    • SIVER
    • GOLD
    • DIAMOND
  • INTEREST FREE DEPOSIT FOR MEMBERSHIP:
    Persons opting for family membership in above categories of membership have to deposit category wise money detailed below with Sanjivani. The money deposited shall not carry any interest:-
    • SILVER: Rs. 2,000/-
    • GOLD : Rs. 5,000/-
    • DIAMOND: Rs.10,000/-
FACILITIES & ASSISTANCE TO THE MEMBER :
Sanjivani shall provide following facilities/Assistance to member-

  1. Medical Treatment, Investigations, Consultation, for medical services at Sanjivani Hospital at Confessional rates given at Schedule: B.
  2. Preference over Non members in admission, allotment of bed facility subject to availability.
  3. Health Check up either Free or at Confessional rates as per Schedule –B
  4. Immunization of children member either free or at Confessional rate as per Schedule –B
  5. Health awareness activities through seminars, magazines, get together etc.
  6. Assistance/ Coordination in arranging treatment in any outside hospital.
  • WITHDRAWAL FROM MEMBERSHIP REFUND OF DEPOSIT:
    Any Member family can withdraw from the membership. On withdrawal of membership, money deposited by the member can be refunded in full after completion of 5 Years only.
  • NOMINEE OF THE MEMBER #
    The member while enrolling for membership has to nominate a person from his family who on his death shall be the authorized person on his behalf.
  • TERMINATION OF MEMBERSHIP :
    The Membership scheme has been launched for extending services & benefits to its member only. If any member is found to be misusing the privilege in any form and in any manner detrimental to the purpose privilege, his membership shall be terminated at the discretion of the management of Sanjivani Hospital. The part of deposited money shall be refunded after deducting the discount given to the person up to the date of Termination.
  • HEALTH CHECK-UP OF MEMBERS :
    Prevention is better than Cure. To this end we wish and desire to help to prevent diseases by having regular Health check-up. Sanjivani will conduct yearly Whole body check-up and Other health checkup of member families at no cost or at confessional rates given at Schedule-B.

  • CEILING AMOUNT OF CONCESSION UNDER CERTAIN CONDITION :
    The Total Concession for various charges in Sanjivani shall be 50% of the amount deposited for Membership that shall be applicable to the member when admitted for the first time if the membership is taken after the admission. Otherwise there shall be no ceiling. For example if the person is admitted on 21 Feb’06 and he takes up the Silver Individual Membership on 22 Feb’06 by depositing Silver Membership Amount of Rs. 3,000/- the maximum discount for all Charges (Hospital , Medicine, Investigation etc. as envisaged in Schedule-B) pertaining to the above admission, shall be limited to Rs 1,500/-. However, the member shall continue to avail full discounts without any ceiling after discharge from the Hospital during subsequent treatments at Sanjivani.
  • REVIEW OF THE SCHEME :
    The scheme may be reviewed every year and any changes shall be intimated to the members. In case the scheme is required to be terminated by Sanjivani the deposited money in full without any interest shall be refunded to the members.

* - Excluding Consultation fee of Consultant/ Doctor concession for which shall be 10% for all categories of members subject to their consent.


@ - If due to non-availability any medicine is purchased  on behalf of or by the patient from any other medical store, no concession shall be given on that medicine.
