FACILITIES & ASSISTANCE TO MEMBER FAMILY:
Sanjivani shall provide following facilities/Assistance to member families:-
- Medical Treatment, Investigations, Consultation, for medical services at Sanjivani Hospital at Cocessional rates given at Schedule: A.
- Preference over Non members in admission, allotment of bed facility subject to availability.
- Health Check up either Free or at Concessional rates as per Schedule–A
- Immunization of children either free or at Cocessional rate as per Schedule –A.
- Health awareness activities through seminars, magazines, get together etc.
- Assistance/ Coordination in arranging treatment in any outside hospital
- Treatment of poor employees of the Member family.

WITHDRAWAL FROM MEMBERSHIP & REFUND OF DEPOSITED : #
Any Member family can withdraw from the membership. On withdrawal of membership, money deposited by the member can be refunded in full after completion of 5 Years only. However, if such member desires to withdraw the money earlier the money shall be refunded with deduction of 25% of the deposited amount.

TERMINATION OF MEMBERSHIP :

The Family scheme has been launched for extending services & benefits to its member families only. If any member is found to be misusing the privilege in any form and manner detrimental to the purpose & privilege, his membership shall be terminated at the discretion of the management of Sanjivani Hospital. The part of deposited money shall be refunded treating this same as a case of withdrawal from membership.
